It has fat droplets in water, kept from separating by proteins and other natural emulsifiers. These help the fat stay mixed in the liquid. Without them, the fat would rise to the top.

Butter forms when more of the water is removed and the fat becomes solid. To picture this, imagine tiny fat droplets floating in water for cream. The result is a solid, spreadable fat.
